Confirmed Exhibitors for SCE ‘Next Steps’ Transition Event

We are delighted to announce the following organisations will be exhibiting at the Social Care Exchange’s  ‘Next Steps’ Transition Event, Home From Home Care are sponsoring on the 28th March, 5pm-7.30pm.

Adults Supporting Adults (ASA) provides support for people over the age of 18 years who wish to remain part of their chosen community. Resources include shared lives (extended stay), day time provision, respite and our day service based in Grantham.

Anthony Collins Solicitors is a specialist law firm with a clear purpose – to “improve lives, communities and society”. For over 40
years they have been successfully combining market–leading legal expertise with a commitment to excellence and long-term relationships. The specialize in advising social care providers nationally, both in the commercial and not-for-profit sectors.

Autism Care UK  is a leading provider of specialist support for people with autism, learning disabilities and associated complex needs. For over 20 years, Autism Care has developed expertise in delivering support pathways that focus on individual outcomes and independence. They work with individuals, their families and other professionals to develop support plans that are person-centred, unique to each person, adaptable and portable. This enables them to regularly review and change an individual’s support package and ultimately be in control of their own life.

Communication Specialist College provide individualised programmes of study in vocational areas are delivered in real work environments. They have a multi-disciplinary approach with on –site therapists and specialist staff for students develop social and independence skills in college and the community.

Diversity Adult Support Services mission is to provide quality-based day opportunities for adults with learning disabilities. They provide tailored individual services, focusing on personal abilities, independence, life skills and choice.

Focus Ability Support And Care Services support people with Learning Disabilities and or Physical Disabilities to learn new skills and reach their goals. Personalisation is at the centre of everything they offer, including; Day Opportunities, Domiciliary Care, Community Supported Living, Respite and Supported Holidays.

Glasshouse Farm Centre is a safe, friendly rural setting, with dedicated buildings, experienced and qualified staff who provide day activities and training for adults with disabilities in Lincolnshire.

Home From Home Care was created by parents to make the difference; Home from Home Care provides specialist residential care for adults (17+) with complex needs. These include Autism, Epilepsy, Cerebral Palsy, PMLD, Sensory Impairment and other associated health and mental health issues, supporting individuals to lead active and fulfilled lives.

JD Pro Development provides multi-sports for disabilities across Lincolnshire working with a variety of disabilities. ‘Aspire’ a day provision open Mon – Sat enabling individuals to access the community and be independent. They are proud to be only UK company to provide a sports programme to disabilities as well as a day provision.

Knowing me Knowing You is a day support service for adults (18+) with learning and physical disabilities in the Lincolnshire area.

Linkage College’s emphasis is on finding the right mix of life skills and study for individuals. Their specialists will support and develop independence in a caring and nurturing environment to deliver the most appropriate learning experience.

Lincolnshire County Council provide information and advice to families of young people in transitions and preparing for adulthood. Works in partnership with schools, colleges, further education providers, adult care, health and other service providers offering support in transitions to adulthood.

Portland College is an independent specialist college for young people with disabilities aged 16 to 25. The college offers education from pre-entry level through to levels 2 or 3. Multi-disciplinary support is provided as well as respite and independent living on their woodland campus in Mansfield, Nottingham.

Say Hello is a new charity that aims to provide services for people in isolation. This could be the disabled, or those who do not fit in with those around them or be looked on as different, resulting in feeling left out and alone. Say Hello will host a variety of events aimed at being accessible to all – including fun runs, cinema showings and coffee mornings. These events aim to remove any prejudice, and be welcoming and encourage integration, or reintegration, with society.

Total Voice delivers free, independent and confidential advocacy. Advocacy can be vital for people who cannot speak up for themselves, or people who feel that they do not have the power to do so. They provide advocates that are training in the Mental Health Act, Care Act, Mental Capacity act and all appropriate
Safeguarding Regulations.

The Thomas Centre is an outstanding Holiday Park for children with Autism and related conditions and for young people and adults in supported living or residential care, with their families or carers.

All exhibitors will be equipped with leaflets, brochures and plenty of information to discuss future options about your child’s future.
