This highly anticipated event, sponsored by Home From Home Care, will be dedicated to showcasing information on transition, specifically what is available for young people once school has finished. Talks will be presented by parents sharing their experiences of transition and key professionals on other relevant topics, as well as information stands from various different providers and services.

It is never too early to start thinking about the future. Transition is a daunting time, however being informed can help make all the difference when it comes to making those important decisions about the future.
